Abstract

PDH) at CV. Permata Store using the Six Sigma method integrated with Failure Mode and Effects Analysis (FMEA). Data were collected through observations, interviews, documentation, and check sheets during the January–March 2026 production period. The analysis employed Defects per Million Opportunities (DPMO), Yield, Sigma Level, Pareto Diagram, Fishbone Diagram, and FMEA. The results showed that the dominant defects were embroidery trapped by the machine and cut finishing. The production process achieved a DPMO value of 87,102, a Yield of 82.58%, and a Sigma Level of 2.86, indicating that the process capability still requires improvement. Based on the FMEA results, the highest-risk failure causes were prioritized for corrective actions. The proposed improvements include operator training, implementation of Standard Operating Procedures (SOP), periodic machine inspections, and appropriate finishing tools. These recommendations are expected to reduce product defects and improve production quality at CV. Permata Store.
